⦁    Applicants should download an LOI proposal form. Please note that the purpose of this LOI proposal is solely to allow for screening of the grant concept; the LOI proposal does not in any manner bind the investigator, Bayer, or the American Academy of Family Physicians in a contractual agreement.
⦁    Applicants should complete the LOI proposal by providing the title of the proposed research or educational project, a 500-word description of the project, and a budget estimate.
⦁    It is necessary to provide the autobiographical details for the investigator. Biographical details for a mentor are preferred but not required.
⦁    Review the application guide or FAQs to ensure that your research is eligible and you have all required materials.
 

Applicants should email completed LOI proposals to the program administrator at REACHAwards@porterhousemedical.com. The deadline for submission is August 16, 2026. LOI proposals will be reviewed by members of the Grants Review Committee. 
 

Research Protocol

⦁    If an LOI proposal is accepted by the Grants Review Committee, a Research protocol form will be emailed to applicants. 
⦁    Acceptance notifications will be provided by September 30, 2026.
⦁    Applicants should complete the Research protocol form and email the document to the program administrator at REACHAwards@porterhousemedical.com.
